Life Insurance

 

If you die while covered under the Health Fund, the Health Fund will pay your beneficiary the amount of death benefits shown on the table below.

 

You can name anyone you want as your beneficiary and you can change your beneficiary at any time. All you have to do is complete, sign and return the appropriate form to the Health Fund Office. Your election must be on file with and approved by the Health Fund Office before you die.

 

If you’re covered as an Active Member and you become totally and permanently disabled before age 62, you may apply to the insurance company to maintain your life insurance — without cost — on a year-to-year basis until you’re no longer disabled or you reach age 65 ― whichever comes first. (This is called “waiver of premium.”)

 

Continuation of coverage in the event of disability is not automatic — you have to provide evidence of your disability to the insurance company each year and get its approval. Contact the Health Fund Office as soon as possible after you become disabled so that you don’t miss the deadline for continuation of your coverage.

 

At age 65, you will no longer be eligible for waiver of premium life insurance coverage and will be offered retiree coverage.

 

Your life insurance benefit may be provided through the Health Fund or from an insurance company. If an insurance company provides it the terms of the insurance company policy also apply. As of July 1, 2001 ULLICO is the provider of life insurance benefits to Health Fund members.

 

You may be able to convert your coverage to a personal life insurance policy if you become ineligible for benefits under the Health Fund. To do so, you must contact the insurance company. It will require you to file an application and pay the first premium within 31 days after your Health Fund coverage ends.
