Welcome to your Prescription Solutions
prescription drug plan offered through Connecticut Carpenters Health Fund. It provides coverage for new and refill
prescriptions filled at participating retail pharmacies and the Prescription
Solutions Mail Service Pharmacy program starting
January 1, 2005.
Just present
your new Prescription Solutions ID card with your prescription to any
participating pharmacy. There’s no need to submit additional claim forms or
paperwork.
Your pharmacy
will enter your Member ID number and other information into their computer
system. This information is automatically sent to Prescription Solutions who
will verify coverage and instruct the pharmacy to collect your applicable
co-payment.
Your
out-of-pocket costs will be less and/or the supply of medication will be
greater if you choose to receive your covered medications through Prescription
Solutions’ Mail Service Pharmacy. For

Your current prescription drug ID card
will no longer be valid starting January 1, 2005.
